29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage.    On or about 05/24/2012, the employer did not ensure that extension cords were protected from physical damage.    Instances:                  a. Extension cords that were run across the first floor were being run over by the various scissor lifts iuse on                   the first fl;oor. As the weight of the scissor lifts is applied to the extension cords, the cords can become                   damaged, exposing internal conductors and expose employees to electric shock.              b. An extension cord under the control of the employer was observed going from the first floor to the second,                  a rise of approx 25 ft, with the cord going over the sharp edge of the 2x4 guardrail at the edge of the                   second floor. The outer insulation of the guardrail could become damaged while the cord rubbed on the                 wood, exposing and  potentially damaging the interior insulation and conductors and exposing employees to                 electric shock.
