29 CFR 1926.1053(b)(16):     Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately  (a) Location: 459 South Main Street, Andover,  MA  On or about 6/20/12, an extension ladder that was damaged was not withdrawn from service.

29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)    (a) Location: 459 South Main Street, Andover, MA  On or about 6/20/12, employees were not protected from falls greater than 6 feet above a lower level while working on a two story roof.    J D General Construction, Inc. was previously cited for violation of this Occupational Safety and Health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA Inspection number 316106681. Citation number 01, item number 003 issued on 12/05/2011, and was affirmed final order on 04/11/2012, with respect to a worksite located at 44 fremont Street, Lowell, MA.
