29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) At the Super 8 jobsite in Brookfield, WI; fall protection was not utilized while performing roofing activities on an about 8/12 roof pitch exposing employees to an approximately 7.9' fall hazard. THE JOSE CRUZ WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1925.501(b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 108284, CITATION NUMBER1, ITEM NUMBER1 AND WAS AFFIRMED AS A FINAL ORDER ON MARCH 12, 2012, WITH RESPECT TO A WORKPLACE LOCATED AT 376 WEILER RD., KAUKAUNA, WI 54131.
