29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    A. Eagle Bridge Co. on or about June 15, 2012 at the address of I-75 & Fourth St., Lima, OH: The employer did not ensure employees were protected from falling approximately 11.00 feet while working on the top of the previously set beams on the west abutment wall. The exposed employee was removing rigging and exposed to fall hazards.     B. Eagle Bridge Co. on or about June 15, 2012 at the address of I-75 & Fourth St., Lima, OH: The employer did not ensure employees were protected from falling approximately 23.00 feet while working on the top of the previously set beams located on the median pier. The exposed employee was removing rigging and exposed to fall hazards.     Eagle Bridge Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501, which was contained in OSHA inspection number 313787491, citation number 2, item number 1 and was affirmed as a final order on July 5, 2011, with respect to a workplace located at Lima, OH.
