29 CFR 1926.502(f)(1)(ii): When mechanical equipment was being used warning lines were erected less than 10 feet (3.1 m) from the roof edge which is perpendicular to the direction of mechanical equipment operation: a) Employees were exposed to the hazard of falling approximately 3 stories where warning lines were erected less than 10 feet from the edge of the roof deck during mechanical equipment operations perpendicular to the roof edge. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or safety monitoring: a) Employees were exposed to the hazard of falling approximately 3 stories while performing roofing activities without the use of fall protection or safety monitoring. A-1 Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10) which was contained in OSHA inspection number 313939654, citation 1, item 1, issued on December 21, 2010, and became final order on or about January 20, 2011. A-1 Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10) which was contained in OSHA inspection number 312599533, citation 1, item 1, issued on January 22, 2010, and became final order on or about February 9, 2010. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
