The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to an employee in that employees were exposed to a fall hazard: a) Warehouse: Employees were exposed to fall hazards when picking stock from tiered shelving units in the warehouse that were 10-20 feet in height. Employees stood on wooden pallets that were sitting on the forks of fork lifts, including, but not limited to, Crown RR5000 series side shift A-56780 serial #1A201979, Crown RR5010-39 serial # 1A192862, Crown RR5020-35 serial # 1A201979, Crown RC4020-50TT188 serial # 9A125987, Crown FC4010-50TT188 serial # 9A129116, Crown C274 model 30SP36TL serial# W-50803, Raymond model EASI serial # EP-B-95-02346, and Raymond EASI R40TT serial # EP-B-98-06735, on or about 7/5/12. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
