29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail systems, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system: (a) Hurst Roofing and Construction at 754 Horizon Drive, Grand Junction, CO 81506: On June 18, 2012 the exposing employer did not ensure their employees were protected from injury, in that four employees installing shingles on the 3-in-12 pitch roof were not utilizing a fall protection system. One employee working at the edge of the roof was wearing a fall harness; however, the lifeline was connected to a side D-Ring on the fall harness. Three other employees were not wearing fall harnesses. This condition exposed employees to a fall hazard of approximately 14 feet.
