On or about June 20, 2012 and times prior thereto, employees of Antonio Elizondo were observed removing shingles from a 5:12 pitch rooftop of a two story residence located at 2525 Millbrook Road in Little Rock, Arkansas.  The employees were not using fall protection, exposing them to a falling distance of greater than six (6) feet to the ground level below.
