29 CFR 1926.501(b)(11):     Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a)  At the jobsite, on the roof, on or about 4/26/12:  Employees were engaged in roofing work on a 6:12 pitch roof at approximately 10 feet and 20 feet ground to eave height, without fall protection of any kind.    Abatement certification must be submitted for this item.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a)  At the jobsite, on the roof, on or about 4/26/12:  Employees were engaged in roofing work on a 6:12 pitch roof at approximately 10 feet and 20 feet ground to eave height, without fall protection of any kind and without being provided training on the hazards associated with falls, or the methods to protect themselves from those hazards.      Abatement certification must be submitted for this item.
