29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about June 20, 2012, employee was operating a Dawoo Lynx 200L CNC Lathe Machine and the door interlock sensor was broken exposing the employee to struck by and amputation hazards. b) On or about June 20, 2012, employee(s) were operating a modified wire brush and sanding machine in which the sanding belt was not guarded which presented an ingoing nip point hazard. c) On or about June 20, 2012, employee was operating a Universal Turret Lathe, no 5, the chuck was not guarded exposing the employees to struck by hazards.
