29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.    On or about 6/7/12, at the Buford job site, employees were exposed to fall hazards up to 18' as they worked from scaffolds that were missing lower guardrails in some areas and upper and lower guardrails in other locations of the scaffold.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.    On or about 6/7/12, at the Buford job site, employees were exposed to fall hazards of 17' as they worked on a 10 in 12 slope roof without the use of fall protection to install stucco materials to an upright column that had been installed extending above the roof.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
