On or about June 15, 2012, an employee in an aerial lift installing exterior masonry caulk did not have a lanyard connected to the basket or boom to protect him from a fall greater than 8 feet to the ground below. NO ABATEMENT DOCUMENTATION OR CERTIFICATION REQUIRED FOR THIS ITEM.
