29 CFR 1910.212(a)(1): One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. a. In the main shop area, an operator was exposed to an ingoing nip point hazard due to the rotating drums on the front side of the Harris L136A printing press being unguarded. b. In the main shop area, operator(s) were exposed to point of operation hazards due to no guarding in place on the 3- Challenge hole punch machines. c. In the main shop area, operator(s) were exposed to the point of operation hazard on the Challenge cornering machine due to no guard in place. d. In the main shop area, operator(s) were exposed to ingoing nip point hazards on the Stahl folder machine due to no guard in place over the feed roller. e. In the main shop area, operator(s) were exposed to point of operation hazards on the 2- Interloch stitching machines due to no guard in place when activating to staple a stack of paper. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or or written records.

In the main shop area, operator(s) were exposed to unguarded pulleys on the Chandler & Price platen press due to no guards in place. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or or written records.

29 CFR 1910.219(e)(1)(i): Where both runs of horizontal belts are seven (7) feet or less from the floor level, the guard shall extend to at least fifteen (15) inches above the belt or to a standard height, except that where both runs of a horizontal belt are 42 inches or less from the floor, the belt shall be fully enclosed in accordance with paragraphs (m) and (o) of this section. a.
