29 CFR 1926.502(d)(6)(i): Unless the snaphook is a locking type and designed for the following connections, snaphooks shall not be engaged directly to webbing, rope or wire rope; a. On or about June 18, 2012 - at the above addressed jobsite, an employee conducting grinding operations from the edge of a building more than twenty five (25) feet above the ground engaged a snap hook to a nylon rope which was tied to a guardrail system of a Genie Z-45/25 Telescopic Aerial Boom Lift, thereby exposing the employee to the hazards associated with falls.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows: a. On or about June 18, 2012 - at the above addressed jobsite, an employee conducting grinding operations from the edge of a building more than twenty five (25) feet above the ground secured his personal fall arrest system to a nylon rope anchor point which was tied in to a guardrail system of a Genie Z-45/25 Telescopic Aerial Boom Lift. This nylon rope anchor point was not capable of supporting at least 5,000 pounds, thereby exposing the employee to the hazards associated with falls.
