29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition: Production Floor, near the Davis Standard Thermatic II plastic extrusion molding equipment: The concrete floor on the west wall of the room was chipped and damaged to the extent that the 600-800 lb. die sets could not easily be moved into position. On or about June 19, 2012, an employee attempted to pull a die cart into position. The wheels of the cart were unable to be pulled over the broken conrete floor to a level surface. The top-heavy cart toppled onto the employee, pinning the employee's legs underneath the equipment.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Mezzanine at the Davis Standard Thermatic II extrusion line was not provided with a standard guardrail, including a toprail and midrail. Employees were exposed to falls of greater than four feet to the concrete floor below.
