29 CFR 1926.451(g)(1):  Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    (a) 41 South Dorcas Street, Lewistown Pennsylvania - An employee worked on a scaffold platform supported by a Pettibone rough terrain forklift.  The employee was not protected from fall with a complete guardrail, personal fall arrest or safety net and was exposed to a fall hazard of approximately 24 feet from the platform to the ground below, on or about June 5, 2012.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    (a) 41 South Dorcas Street, Lewistown Pennsylvania - An employee installed roof sheathing along the rake edge of the roof with a 6 in 12 pitch.  The employee was not protected from falls with a guard rail, safety net or personal fall arrest harness, and was exposed to a fall of approximately 27 feet from the rake edge to the ground, on or about June 5, 2012.    MIDDLE CREEK CONTRACTORS WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314244682, CITATION NUMBER 1, ITEM 2 AND WAS AFFIRMED AS A FINAL ORDER ON JUNE 13, 2011, WITH RESPECT TO A WORKPLACE LOCATED AT HUNT PLACE, MECHANICSBURG, PA 17055.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
