29 CFR 1910.146(k)(3)(ii): Whenever an authorized entrant entered a permit space to perform rescue services, a mechanical device was not available to retrieve personnel from vertical type permit space more than 5 feet (1.524 m): At this establishment, employees entered the inside of frac tanks, which are greater than 5 feet in height, to clean using spray guns with high pressures of approximately 2500-3500 psi, and the employer did not have a rescue tripod or other device available to perform rescue operations.
