29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) On or about June 14, 2012, Wells & West General Contractors, as the controlling and creating employer, did not ensure that Colorado Framing & Construction LLC, a sub-contractor, protected their employees from electrical hazards while using electrical extension cords connected to the tools (circular saw, compressor) where the electrical cords did not have a ground plugs.
