1926.100(a) Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.      Worksite, West - Employees were observed working under other workers in areas where objects such as, but not limited to, hand tools, containers of stucco and other finishing supplies could potentially fall on, or about, 06/15/12.        Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    Worksite, east - Employees were working on a scaffold at the heights of approximately 12 feet. The scaffold was not fully planked on, or about, 06/15/12.    Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.    Worksite, east - Employees engaged in stucco work on a commercial building were observed working on a scaffold approximately 12 feet above the ground. Personnel were accessing the top platform by climbing the scaffold. There was no safe access on, or about, 06/15/12.         Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
