29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a. Entire facility - On or about June 20, 2012, the employer had not implemented an energy control program for employees who perform servicing and maintenance on such equipment including but not limited to, the vertical and horizontal hobbing machines. Employees were thereby exposed to the hazards associated with the unexpected energization of equipment.

29 CFR 1910.219(f)(4): Openings for oiling. When frequent oiling must be done, openings with hinged or sliding self-closing covers shall be provided. All points not readily accessible shall have oil feed tubes if lubricant is to be added while machinery is in motion. a. Large Hob Department - On or about June 20, 2012, an employee experienced an amputation injury while adding lubricating oil to a vertical gear hobbing machine's oil reservoir (#96 Gould & Eberhardt). The machine was in motion, thereby exposing employees to the hazards associated with unguarded in running nip points.
