29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a) Residential Home Side - On or about 21 June, 2012 - Employees were cleaning up around the base of the home without wearing hard hats while employees were working overhead installing roofing shingles. Employees were exposed to a struck by hazard.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)  Note:  If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502.    a) Residential Home Roof - On or about 21 June, 2012 - Employees were installing asphalt roof shingles on an 8 in 12 pitch roof without the use of conventional fall protection. Employees were exposed to a fall hazard of approximately 24 feet.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder.    a) Residential Home Front - On or about 21 June, 2012 - Employees were accessing the upper landing with a ladder that did not allow for the side rails to extend a minimum of 3 feet above the landing being accessed Employes were exposed to a fall hazard of approximately 28 feet.
