In the operation of the rotary boiler, at 800 Main St., Dalton, MA:    On 6/17/12 and at times prior, employees who loosened the head cover yokes on the rotary boiler did so without an accurate hazard assessment which would necessitate the use of personal protective equipment and were exposed to burn hazards from the hot water vapor released from the boiler.
