29 CFR 1926.20(b)(2): 29 CFR 1926.20 (b)(2) The employer did not develop an accident prevention program, that provided for frequent and regular inspections of the job sites, materials, and equipment, to be made by competent persons designated by the employer. a) 3 Azalea Drive, Rumson, NJ: The employer did not develope a program that provided for frequent and regular inspections of the jobsite fort he employees applying sheathing at a height greater then 11 feet from the ground level resulting in a injury from a fall from heights, observed on or about 6/18/12. b) 3 Azalea Drive, Rumson, NJ: The employer did not develope a program that provided for frequent and regular inspections of the jobsite fort he employees applying sheathing at a height greater then 11 feet from the ground level. An employee on the day of the inspection was not utilizng fall protection while working on from the roof, observed on or about 6/18/12. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

An employee worked from plywood stacked on the forks of the lift and were not secured, observed on or about 6/18/12. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

The employee was applying the sheathing to the roof when he slipped and fell, observed on or about 6/18/12. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.
