General-duty citation text
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by and fall from elevation hazards associated with the collapse of Long Span wood trusses: On 06-08-12 and 06-19-12, trusses of a new commercial building, Windsor Center, under construction suffered two separate partial collapses with a total of more than sixty (60) long span trusses which had collapsed. Employees who were installing sixty (60) feet L trusses on 06-08-12 and were installing additional truss bracing and sheathing on 06-19-12 were exposed to struck by and fall from elevation hazards associated with improperly braced trusses. Among other methods, a feasible and acceptable means to correct these hazards are to follow the instructions and notes on the plans provided by the truss engineer and the truss fabricator which reference Long Span Truss Installation safety instructions in the Structural Building Components Industry and the Building Component Safety Information (BCSI-2008) manuals for installing temporary and permanent truss bracing. Abatement Certification and Abatement Documentation required
