29 CFR 1926.28(a):     Appropriate personal protective equipment was not worn by employee(s) in all operations where there was exposure to hazardous conditions:    a) On or about 6/19/12 in the Boiler Room at Sutherland Global Services Rochester, New York;  employee removed the front protective covers on Section 1 of the ( 480Y/277V, 3 phase 4 wire 1600 amp ) Challenger "Power Master" switchboard exposing energized electrical parts and was required to wear a faceshield, flame resistant clothing, hearing protection and voltage rated protective gloves prior to placing a 30 amp circuit breaker far enough into the #3 slot on the energized switchboard causing an arc flash/ arc blast. 29 CFR 1926.416(a)(1):     Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    a) On or about 6/19/12 in the Boiler Room at Sutherland Global Services Rochester, New York; employee removed the front covers for circuit breakers #2 & #4 on Section 1 of the ( 480Y/277V, 3 phase 4 wire 1600 amp ) Challenger "Power Master" switchboard exposing energized electrical parts without providing any type of protective cover or guarding against contact with these parts. 29 CFR 1926.416(a)(3):     Employer did not advise employees of the the hazards involved and the protective measures to be taken:    a) On or about 6/19/12 in the Boiler Room at Sutherland Global Services Rochester, New York; employee (1) removed the three front covers on Section 1 of the ( 480Y/277V, 3 phase 4 wire 1600 amp ) Challenger "Power Master" switchboard and began to install a 30 amp circuit breaker into the #3 slot of the energized switchboard when an arc flash/arc blast occurred.  Employee (2) was assisting employee (1) by holding a flashlight to illuminate the work area in proximity to the energized switchboard.  Employee (1) did not advise employee (2) about the arc flash/arc blast hazards of working on or near energized electrical equipment nor the protective equipment to be worn.
