29 CFR 1910.305(a)(2)(ix):     Lamps for general illumination were not protected from accidental contact or breakage by a suitable fixture or lampholder with a guard:    On or about May 22, 2012, inside the paints and chemicals storage shed, a fluorescent light fixture at 120 volts within reach of the work floor was not provided with guarding of live parts. 29 CFR 1910.146(c)(8)(i):     Inform the contractor that the workplace contains permit spaces and that permit space entry is allowed only through compliance with a permit space program meeting the requirements of this section:    On or about June 21, 2012, and at other times prior thereto, at this site, the employer did not develop a confined space program and coordinate the requirements of the standard (host employer) with visiting contractors performing work, including but not limited to, storage tank maintenance.
