29 CFR 1926.404(f)(6):  The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    Third floor, south wall area - Employees were using portable extension cords, to power electrical equipment, that had the ground pin missing.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    a) South Corner of Roof - Employees were standing on the top plate installing trusses without a personal fall arrest system in place to prevent them falling 35 to ground level.    b) Third floor, South Wall - Employees were working near open wall openings that was not equipped with a top rail measured vertically at 42 inches.  Employees were exposed to a fall from elevation of approximately 35 ft. to the ground below.

29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) Third Floor, South wall - Employees were using a step ladder, to access the top plate area, that was flush to the landing platform.
