Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    On or about June 21, 2012, employees were observed working near an unguarded side or edge of a building without fall protection at heights greater than 30 foot, exposing employees to fall hazards.
