29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: A. Mike Ireland worksite located at 8167 S. Bridge Way, Maumee, Ohio: On or about June 22, 2012 the employer did not ensure employees wore proper eye protection while operating pneumatic nail guns while performing weatherproofing activities on the two story residential structure. The employees were exposed to potential flying object hazards while operating the pneumatic fasteners.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: A. Mike Ireland worksite located at 8167 S. Bridge Way, Maumee, Ohio: On or about July 2, 2012, the employer did not ensure employees were protected from falls while performing weatherproofing activities on the two story residential structure. The employees were exposed to a fall distance of approximately 12.00 feet. Mike Ireland was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501, which was contained in OSHA inspection number 313787970, citation number 1, item number 1 and was affirmed as a final order on September 15, 2011, with respect to a workplace located at Monclova, Ohio
