29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:  (a)On or about June 21, 2012, an employee was exposed to fall hazards when the bakers scaffold he was using moved while he was climbing on to it because some of the wheels locks were not applied and others were broken, inside building 5 at an apartment complex under construction at 16173 Old Forest Pt. in Monument, Colorado.    Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
