29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: The employer does not ensure that a body belt and a lanyard are worn and attached to the boom or basket when working from an aerial lift. This violation occurred on or about June, 18, 2012, near the corner of Yale Street and Center Street where employees were exposed to a fall hazard of approximately 25 feet without using a fall protection system.
