29 CFR 1926.652(b)(4)(i):  Sloping and benching for excavations greater than 20 feet were not designed by a registered professional engineer:     (a)  Beavers Construction Co. at 1950 Rd, Austin, CO 81410:  On and before June 19, 2012, the exposing employer did not ensure that each employee working within an excavation that was approximately 90 feet in depth, with an opening diameter of approximately 280 feet, was protected from cave-in, in that the sloping protective system used was not designed by a registered professional engineer. The top section of the excavation was undercut causing employees to be exposed to a crushing hazard by excavation wall cave-in.
