29 CFR 1926.501(b)(7)(ii): Each employee at the edge of a well, pit, shaft, or similar excavation 6 feet (1.8 m) or more in depth was not protected from falling by guardrail systems, fences, barricades, or covers: On or about June 19, 2012, employees assisting in foundation shaft drilling and clean out work using a Texoma 600 Drill Rig were not protected from falls. Employees were exposed to fall hazards of approximately (27) to (30) feet into the shaft below.
