29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(d)(2) in that the equipment used for protective systems was not used in a manner that was consistant with recommendations of the manufactirer, and was not used in a manner that will prevent employee exposure to hazards: a) Trench on Jeanette Street - Employees who entered the 12 foot deep and 7 1/2 foot wide trench to join the pipes were not provided with adequate trench boxes or other systems to protect the access ramp and the trench area while they were working in the trench thereby exposing the employees to a cave-in hazard, observed on or about 6-21-12. Abatement certification required
