29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    At the worksite, on June 20, 2012, employees conducted maintenance and repairs on the various pieces of equipment such as but not limited to the bending machines and the shears.

29 CFR 1910.212(a)(3)(ii):     Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:    At the worksite, on June 20, 2012, the gear for the Waterburry Farrel threading machine was not guarded.  This condition exposed employees to caught-by hazards.

29 CFR 1910.219(d)(1):     Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):    (A)  At the worksite, on June 20, 2012, the pulley for the 25 ton Alva Allen Ind. model number BT25 shear was not equipped with a guard.  This condition exposed employees to caught-by hazards.     (B)  At the worksite, on June 20, 2012, the pulley for the 12 ton Alva Allen Ind. model number BT12 shear was not equipped with a guard.  This condition exposed employees to caught-by hazards.

29 CFR 1910.303(g)(1)(i)(B):     For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the width of the working space in front of electrical equipment was less than the width of the equipment or 762 mm (30 inches), whichever was greater :    (A)  At the worksite on June, 20 2012, the fuel gas cart blocked access to the circuit breaker panels located on the central north facing wall.  This condition exposed employees to electrical shock and burns.      (B)  At the worksite on June 20, 2012, the Landis grinder blocked access to the circuit breaker panels located on the central north facing wall.  This condition exposed employees to electrical shock and burns.      (C)  At the worksite on June 20, 2012, the metal barrel blocked access to the circuit breaker panels located on the centra north facing wall.  This condition exposed employees to electrical shock and burns.

29 CFR 1910.178(l)(1)(ii):     The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a forklift:    At the worksite, on June 20, 2012, the two employees who operated the CAT model C5000 S/N AT9003251 and the CAT model P5000 S/N AT3501030 forklifts, had not recieved forklift training.
