29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On 06/13/2012, employees engaged in building framing activities were exposed to electrical hazards while using the energized portable cords/extension cords not provided with strain relief protection and whose individual conductors were pulled loose from the plug. The following energized 3 conductor portable cords with missing strain relief protection operated at 120 Volts AC and was resting upon the workplace floor: a. Yellow colored, 3 conductor extension cord, b. Blue colored with Red Stripe, 3 conductor extension cord.
