29 CFR 1926.405(e)(1):     Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations were not installed so as to prevent moisture or water from entering and accumulating within the enclosures:    North side exterior area of the workplace, temporary electrical power pole, receptacle cover: On 06/13/2012, employees engaged in construction activities were exposed to shock hazards from contact with energized receptacles installed within a weatherproof box that did not have a cover. The box was installed within a damp location at a height of 36 inches above the ground; and contained receptacles that operated at 120 volts AC.
