29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.     On or about 6/19/2012, at the Commerce job site, employees performing service and maintenance to equipment were not trained in the company's Lockout/Tagout Program as Authorized employees in that "Sewers" were changing needeles on the sewing machines, electrically powered, but were trained as Affected employees.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.147(c)(7)(i)(B): Affected employees were not instructed in the purpose and use of the energy control procedure.    On or about 6/19/2012, at the Commerce production facility, two newly hired employees working in the production room were not provided Affected person training in the company's Lockout/Tagout program prior to being exposed to activities involving lockout/tagout work that was being conducted in the facility. Energy sources include electrical, pneumatic and hydraulic energies.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed.    On or about 6/19/2012, at the Pignone Fast T Looms, the chain and sprocket located on the motor at the cord frame was not guarded and was 6'5" from floor levels. Motors were left unguarded on looms #2, #6, #7 and #8.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area.    On or about 6/19/2012, at the Commerce production facility, emmployees were in contact with lubricant oils and greases, and liquefied petroleum gas in cylinders (LPG), and the employer did not train the employees in relation to the Hazard Communication Program.     The employer was issued a citation addressing this deficiency in inspection #315739870, citation 1 item 2b, issued on 2/2/2012. The employer provided documentation of abatement dated 3/13/2012 to certify that training had been provided to employees at the facility. However, training was not conducted at the faciliity to production employees and the employer failed to abate the citation as documented on 6/19/2012 in follow up  inspection 480751.      No abatement certification or documentation is required for this item. The training occurred on June 20th, 2012.
