29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  The employer does not ensure that each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vii) of this section, more than 10 feet above a lower level is protected from falls by the use of a personal fall arrest system or guardrail system meeting the requirements of paragraph (g)(4).  This violation occurred on or about June 18, 2012, on the east side of the commercial building where an employee was exposed to fall hazard of more than 10 feet when working from the top rails of a JLG scissor lift to install wooden boards for framing without a fall protection system.

The employer does not ensure that guardrail systems are installed along all open sides and ends of platforms.  This violation occurred on or about June 18, 2012, on the east side of the commercial building where an employee was exposed to fall hazard of more than 10 feet when working from a JLG scissor lift to install wooden boards for framing without guarding one end of the scissor lift.

The employer does not ensure each employee on a walking/working surface with an unprotected side or edge that is 6 feet or more above a lower level is protected from falling by the use of a guardrail system, safety net system or personal fall arrest system.  This violation occurred on or about June 18, 2012, on the east side of the commercial building where an employee was exposed to fall hazard of more than 6 feet when framing the building without a fall protection system.
