29 CFR 1926.403(b)(1):     Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:                                                a)  21315 IH-10W, San Antonio, TX,  On or about 06/20/12, and at times prior thereto, employees were exposed to electrical hazards while connecting and using an yellow extension cord which did not have effective strain relief.    b) 21315 IH-10W, San Antonio, TX,  On or about 06/20/12, and at times prior thereto, employees were exposed to electrical hazards while connecting and using a 3-way GFCI extension cord which did not have effective strain relief.
