29 CFR 1910.332(b)(2): Employee, who were not qualified persons, were not trained in and familiar with any electrically related safety practices not specifically addressed by 29 CFR 1910.331 through 29 CFR 1910.335, but which were necessary for his safety.    a)  Fire Pump Room:  Employee, not trained in electrically related safety practices, was exposed to an arc flash explosion while engaged in work activities in close proximity to an energized 480V 3 phase electric fire control panel box while it was being serviced, on or about 6/18/2012.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed.    a)  Fire Pump Room:  Employee, not wearing electrical protective equipment, was exposed to an arc flash explosion while engaged in work activities in close proximity to an energized 480V 3 phase electric fire control panel box while it was being serviced, on or about 6/18/2012.
