29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: a) On or about 6/15/12 at the Ramada Inn Fitness Center at 455 US Highway 41 Bypass North, Venice, FL a 4' x 4' hole cut for a skylight was covered by an unsecured piece of plywood. An employee for a subcontractor, Flores Bros. Inc. removed the plywood and fell through the hole - 14' to the concrete floor below.

The cover was not coded or marked with the word HOLE or COVER. An employee for a subcontractor, Flores Bros. Inc. removed the plywood and fell through the hole - 14' to the concrete floor below.
