The employer does not require the wearing of appropriate personal protective equipment such as seat belts on material handling equipment exposing employees to injury.  This violation was observed on or about June 20, 2012, on the east side of the jobsite between Buildings 6 and 7 where an employee was exposed to a crushed-by hazard when operating a JLG Skytrak Telehandler Model 10054 (SN 87022) fork truck without using a seat belt.
