29 CFR 1910.23(c)(1):  Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent):    400 5th St., Utica, NE 68456 (Outside Concrete Bin Platform): Employees were accessing the top of a platform during the loading of concrete trucks without fall protection. The platform was approximately 10 feet in height.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed.
