29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    a)  Gutierrez, Carlos at 614 Sparrow Place, Fort Collins, CO 80525:  On or before June 23, 2012, the employer did not ensure that an employee wore safety glasses when working in the vicinity of pneumatic nail guns.  Condition exposed this employee to the hazard of projectiles to the eyes.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    a)  Gutierrez, Carlos at 614 Sparrow Place, Fort Collins, CO 80525:  On or before June 23, 2012, the employer did not ensure that an employee was wearing fall protection when shingling a residential roof with up to an 7 in 12 pitch.    Condition exposed this employee to fall hazards of up to 12.3 feet.
