29 CFR 1926.451(g)(1):     Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:                                                                                                        a)  Corner of FM 78 and Lakeview Dr., San Antonio, TX,  On or about 06/22/12, and at times prior thereto, employees were exposed to the hazards of falls from an elevation of 37' 10"  to the concrete ground below while climbing the mid and top rails of the scissor lift, which was used to accessing the roof.
