29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a) Diablos Construction at 5126 Stardust Lane, Fort Collins, CO 80525: On or before June 23, 2012, the employer did not ensure that an employee wore safety glasses when using or working in the vicinity of pneumatic nail guns. Condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: a) Diablos Construction at 5126 Stardust Lane, Fort Collins, CO 80525: On or before June 23, 2012, the employer did not ensure that an employee was protected from falling when a pump jack scaffold was elevated in the center to side the peak of a 2 story house, and the scaffold had no end rails. The employee was not using a personal fall arrest system. Condition exposed employees to fall hazards of about 20 feet.
