29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6). Note:  Specify distances as indicated in table in paragraph (f)(6) of this section.     a) Jobsite - 409-11-13 Manton Street -The clearance between the tubular welded frame scaffold and the insulated Tri-plex type overhead power lines was less than three (3) feet potentially exposing employees to electrical hazards.  Observed on or about 6/22/12.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:    a) Jobsite: 409-11-13 Manton Street, Philadelphia, PA- the working platforms on the tubular welded frame scaffold were missing planks.  Employees were performing masonry operations up to 19.5 feet above ground level.  Observed on or about 06/22/12.    DORIAN MASONRY WAS PREVIOUSLY CITED FOR THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD WHICH WAS CONTAINED IN OSHA INSPECTION #315819383, CITATION 1 ITEM 1 AND WAS AFFIRMED AS A FINAL ORDER ON JUNE 15, 2011 WITH RESPECT TO THE WORKSITE LOCATED AT 8TH & CHRISTIAN STS., PHILADELPHIA, PA.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.    a) Jobsite: 409-11-13 Manton Street, Philadelphia, PA- the tubular welded scaffold did not have any portable ladders or other means of access to the working levels of the scaffold.  Observed on or about 06/22/12. 29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means:    a) Jobsite: 409-11-13 Manton Street, Philadelphia, PA- the working platforms on the tubular welded frame scaffold were up to 26 feet high and the working levels were up to 19.5 feet, performing masonry operations.  There were no restraints to prevent the scaffold from tipping. Observed on or about 06/22/12. On or about 06/22/12.
