29 CFR 1910.36(d)(1):     Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:      On or about June 25, 2012, October 23, 2012 and at times prior thereto employees were possibly not able to open the south exit door of Building 327 from the inside at all times without keys, tools, or special knowledge because the door-knob lock device could be accidently or intentionally engaged. This exposed employees to a possible fire of greater intensity increasing the possibility of more severe burns, an increased amount of smoke inhalation and impaired visibility due to delay or the inability to unlock the door.

29 CFR 1910.37(b)(2):     Each exit was not clearly visible and marked by a sign reading "EXIT"    On or about June 25, 2012,October 23, 2012 and at times prior thereto the south and east openings into an exit of Building 327 were not marked by a sign reading "EXIT". This exposed employees to a possible delay in finding or inability to find an exit opening therefore increasing the possibility of more severe burns and increased smoke inhalation.     Location:   a) South exit door of bldg. 327  b) East exit door of Bldg. 327

29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    Instance a) On or about June 25, 2012 in the blow mold department, employees operating the bottle leak detector machines were exposed to being struck by rotating parts because the front doors of the machine enclosure were missing.    Instance b)  On or about June 25, 2012 in the blow mold department, employees operating the Conair#1 re-grinder machine (hog grinder) were exposed to possibly being struck by the rotating parts at the point of operation should they unbolt and open the top barrier guard or back gate of the grinder while it is operating because the interlock was defective.

29 CFR 1910.22(a)(1):     All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.     On or about June 25, 2012 in the trimmer area of the blow mold department, two extension cords supplying power to portable fans were lying on the floor which exposed the cords to damage by being run over by forktrucks and created a possible tripping hazard to employees walking/working in the area.
