29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter:    (a) Facility: The employer failed to provide training on the use of fire extinguishers and the hazards involved in fighting incipient stage fires upon initial hiring and at least annually thereafter, occurred on or about 6/20/12.    BY THIS DATE THE EMPLOYER MUST EITHER CORRECT THE ALLEGED VIOLATION(S) OR IMPLEMENT A FIRE SAFETY POLICY, AS OUTLINED IN 29 CFR 1910.38 WHICH INCLUDES THE EVACUATION REQUIREMENTS OF 29 CFR 1910.157(b).    NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.

29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following:    (a) Facility: The employer did not develop and implement a written hazard communication program for employees working with and around, products including but not limited to, stainless steel cleaner, Clorox clean-up, simple green and scrubbing bubbles bathroom cleaner, 6/20/12.    NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
